What Is Giant Congenital Melanocytic Nevus?
GCMN is essentially a large mole present at birth. It forms between 5 and 24 weeks of pregnancy, and the earlier it develops, the larger it can grow. Though it may shrink or grow over time, giant nevi tend to expand as the child ages. Besides the potential health risks, the unique appearance of GCMN can also affect a child’s self-esteem.
“Beyond its unusual appearance, it poses a risk of malignant degeneration. The cells in the affected skin are abnormal and may develop into melanoma, a highly aggressive type of skin cancer,” explained Dr. Anthony Wolfe, a pediatric plastic surgeon at Nicklaus Children’s Hospital.
Exploring Treatment Options
Treatment for GCMN depends on the size and location of the birthmark, as well as the child’s age. Options include:
- Dermabrasion: A wire brush or diamond wheel removes layers of skin, lightening the nevus but potentially leaving scars. This is most effective within the first six weeks of life.
- Skin Curettage: Scraping the top layers of skin, also best performed in the first six weeks of life.
- Tangential Excision: A blade removes the top skin layers to reduce the nevus’s visibility, but complete removal is rare.
- Chemical Peels: These can improve the appearance of lighter-colored nevi, using agents such as phenol or trichloroacetic acid.
For Luna, none of these options in the U.S. offered an ideal solution. Her parents wanted to remove the birthmark before she started school to prevent bullying but struggled to find an effective treatment. “We consulted doctors in Boston, Chicago, and New York, but we couldn’t find the results we hoped for,” Carol shared.
A Lifeline Through Instagram
Carol turned to social media to share Luna’s story, and the response was overwhelming. Among the many messages was one from Dr. Pavel Popov, a Russian laser and cancer specialist with over 20 years of experience and more than 1,000 surgeries to his name. He introduced Carol to a cutting-edge treatment called photodynamic therapy, not yet available in the United States.
