The policy that Caddo Springs adopted in three minutes in June 2012 came with a form.

It is called a Student Bullying Incident Report.

It is one page, front and back.

It has a district logo on it and a revision date in the bottom corner.

It has spaces for the reporting party, the date, the students involved, a description, and a section headed ACTION TAKEN with six checkboxes and a line for “other.”

And near the bottom of the reverse, in a block of four small boxes, there is 7b.
